GUIDE Dementia Support
More support for people living with dementia and the families who care for them
Caring for someone with dementia can bring new questions and challenges at every stage. Hillview Adult Day Health Center is pleased to be involved in the GUIDE program, which helps eligible people living with dementia and their caregivers connect with coordinated care and practical support.
GUIDE stands for Guiding an Improved Dementia Experience. It is a Medicare program designed to improve dementia care, support family caregivers, and help people remain in their homes and communities for longer.
What support does GUIDE offer?
Depending on a person’s needs and eligibility, GUIDE can include:
• A personalized dementia care plan and help coordinating care
• A care navigator to connect families with services and local resources
• Education and support for family caregivers
• Access to a support line at any time
• Respite services for qualifying families, giving caregivers time to rest or attend to other responsibilities
At Hillview, we understand how valuable a welcoming, structured day can be for someone living with dementia—and how much a dependable break can mean to a caregiver. Our team can talk with you about Hillview’s involvement in GUIDE and help you understand the next step.
Who may qualify?
GUIDE is for people with a dementia diagnosis who have Original Medicare Parts A and B as their primary coverage and meet the program’s other requirements. People enrolled in Medicare Advantage or PACE, or who have elected the Medicare hospice benefit, do not qualify. The GUIDE care team and Medicare confirm eligibility.
There is no patient cost sharing for GUIDE services, including approved respite services. The services available to each person depend on their assessment and the program’s requirements.
